Observation

Licensor spoke with staff at the school age program, including the director and the staff member involved in serving the granola as well as the mother of the child affected. Through these interviews it was clear that the staff member did not follow proceedures in 110-300-0186(4) , reviewing the care plan for allergies and 110-300-0186(3)(b) . This intake will be considered Valid by the licensor and submitted to the licensing supervisor for approval.
